The Essence of Vanity is a possible drop when defeating high level enemies, such as Nightmares and other powerful targets, on The Moon. Once obtained, it goes into your quest log as a new quest with several objectives including:

The first two objectives are easy enough to understand but if you want to find the Horned Wreath, you need to travel to the Hellmouth area on The Moon. You’ll see a large doorway, enter the doorway.
Once inside, take a left down the stairs. You must follow the path all the way to the end. It’s a very long dungeon, lots of rooms, tons of enemies. It will probably take you an hour or two to fight through it, but you can just run through and avoid it as long as you’re not too low a Light Level. At the very end of the dungeon there is a large room, a single Ogre stands at the back. As you approach, the Horned Wreath quest icon will appear on screen. Follow it to grab the item.
