Unlike most of the previous Essence items, the Insanity Essence doesn’t appear to drop from random enemies. Head to this location in Hellmouth on the Moon. There’s a large passageway lit green. Head inside and take a left. Then follow the path deeper into the area until you reach The Gatehouse. Once you reach The Gatehouse keep progressing until you see this room.
Inside this room there is a mini-boss called Ehrathur External Blade. Kill him. Once you have killed him, he will drop the Bound Manacle.
